On April 15, 2024, the Delaware Department of Insurance (DEDOI) released Auto Bulletin No. 35 informing all Delaware licensed insurers, brokers, and agents providing automobile coverage in Delaware that new financial responsibility forms are available on the Delaware Division of Motor Vehicles (DMV) website. The provided new forms are:

• FR1 - Delaware Uninsured Vehicle Report
• FR4 - Notice of Cancellation or Termination of Policy
• FR19 - State of Delaware Insurance Certification

Effective 90 days from the date of issuance of the bulletin, the use of these forms will be mandatory and the DMV will no longer accept the information provided via these forms in any other format.
